NEW Students to Miami-Dade County Public Schools.
MUST present the following documentation to REGISTER student at American Senior High. 

  1. Proof of address – Please provide (2) of the following documents below: 
    NO OTHER DOCUMENTS CAN BE ACCEPTED 
    • Warranty Deed or Notarized Lease Agreement and 
    • Most current FPL Bill 
  1. Student’s ORIGINAL Birth Certificate 
  1. Immunization Records (converted to Florida form #680 blue form – full page version is the only form allowed) 
  1. Physical Exam Record (Including Tuberculosis Test results) yellow form 
  1. School Records from previous school (documents other than English, Spanish, or Creole must be translated to English) 
  1. Parent Picture Identification 
  1. Completed registration forms/packet by opening the link for each form required, save them and send them to aalvarez08@dadeschools.net or 277502@dadeschools.net  

You will receive an email confirmation on registration or pending information not received once we receive all documentations and forms required. 


TRANSFERRING Students from another Miami-Dade County PublicSchool. 
MUST present the following documents to REGISTER: 

  1. Proof of address – Please provide (2) of the following documents below:

    • Warranty Deed or Notarized Lease Agreement and

  1. Withdrawal papers from the previous Miami-Dade County Public School 
  1. Parent Picture Identification 

WITHDRAWING Students to another MDCPS school or other school.
MUST present the following documents to REGISTER: 

  1. Proof of new school to serve student. 
  1. Completed Withdrawal form  
  1. Parent Picture Identification 
  1. Parent who registered student MUST be present at the time of Withdrawal
