NEW Students to Miami-Dade County Public Schools.
MUST present the following documentation to REGISTER student at American Senior High.
- Proof of address – Please provide (2) of the following documents below:
NO OTHER DOCUMENTS CAN BE ACCEPTED- Warranty Deed or Notarized Lease Agreement and
- Most current FPL Bill
- Student’s ORIGINAL Birth Certificate
- Immunization Records (converted to Florida form #680 blue form – full page version is the only form allowed)
- Physical Exam Record (Including Tuberculosis Test results) yellow form
- School Records from previous school (documents other than English, Spanish, or Creole must be translated to English)
- Parent Picture Identification
- Completed registration forms/packet by opening the link for each form required, save them and send them to aalvarez08@dadeschools.net or 277502@dadeschools.net
You will receive an email confirmation on registration or pending information not received once we receive all documentations and forms required.
TRANSFERRING Students from another Miami-Dade County PublicSchool.
MUST present the following documents to REGISTER:

WITHDRAWING Students to another MDCPS school or other school.
MUST present the following documents to REGISTER:
- Proof of new school to serve student.
- Completed Withdrawal form
- Parent Picture Identification
- Parent who registered student MUST be present at the time of Withdrawal
